If it's a mirror link, it's just the same file from a different server. If it's a game with multiple disks, you need to download every disk to have the complete game. If it's just scrubbed vs. unscrubbed, nothing (in any practical sense as far as the user is concerned) except how big the file is. If it's something in the emulation section, it's just the version of the emulator for a specific OS (Windows/Mac/Linux/etc.).
Whats the Difference Between the two download links?
