Note: Missing empty folders are "automatically added in" when rebuilding an ISO with an IRD file, so it is typically better to build your ISOs this way when possible.

Check if there is an empty folder called "system" in /PS3_GAME/USRDIR/. If it's not, add it and try to run your game again.

Also, it is typically better to run dumps as ISOs rather than in folder format, so if you haven't already try converting the game using makeps3iso.

Building an ISO with an IRD file might also give better results. Use PS3-ISO-Rebuilder to do so (irds can be found here: http://ps3.aldostools.org/ird.html; they are for now unavailable in the Lair).
