Well, now it's clear why there was a duplicate here.
Some PS3 games were re-printed with different firmware on the disk, yet both games are the same version so The Vault can't tell them apart. I think I can solve that by appending the firmware version to the game version. The database is quite upset about making that change mid-day though, so hopefully it settles down soon.
