I believe they're referring to the ROM hacks you can get RetroAchievements for, or maybe the subsets?
In both cases, the patch is usually available directly from RA on the "Supported Game Hashes" page. Just grab the original ROM from the Vault and apply the patch to the ROM using Rom Patcher JS (or your other patching tool of choice).

Which ROMs are you referring to? RetroAchievements doesn't normally require a ROM patch to use.
