There's a second phone jack, but no difference. Connecting straight into the demarc didn't fix it either. I swapped out the cords and cleaned the connectors with DeoxIT, all to no effect.

Is there only one phone jack in the house? You might try an alternate jack and see if its performance is the same.
I'd also try checking your modem drivers. I know the drivers can get corrupted for a number of reasons, usually through no fault of the user (Windows being the incompetent dumbbell it is, usually).
