How do i upload?

I've used this site a long while and I have 2 PS2 games that are not in the vault.
I can't seem to find a guide or anything on how to upload. Could anyone give me a walkthrough if there isn't already one floating around?

Questions like,
-what software do I rip the games with?
-how do I do MD5 Checksums?
-Am I supposed to upload a rar file with .bin and .cue files or an ISO?
